The banners are not final copies. Please approve your photo or request to change it via the form below. No other design changes or special requests will be approved. 

After viewing your banner, click the link to the approval form below and complete the form. You can approve or change your photo and also input your name as it should read on the banner. It is  quite simple but if you have any questions, please feel free to have a parent or guardian reach out to us and we will get it taken care of. Requests for photo change on your banner must be completed no later than 6 pm on Sunday, January 9th.
